KAPA Application Process
- For Knoxville Area Project Access (KAPA), use the new simplified online form: Family Health Administration System – Electronic Services (fhases.com) or call the Knoxville office (865-531-2766) and they will fill out the form for you.
- They will need: Name, address, county they reside in, telephone number, date of birth, medical service need and name of the referring Lions Club member. That will start the process and KAPA will respond quickly.

Appalachian Mountain Project Access (AMPA)
- KAPA also works with Appalachian Mountain Project Access (AMPA) in Johnson City and will refer patients there as appropriate depending on patient location and services needed.
